Forms that have been assigned to a student-athlete can be found on the My ARMS -> My Dashboard page underneath the "FORMS REQUIRING IMMEDIATE ATTENTION" section.

From there, click on the name of the form to launch it.

Once the student-athlete has completed all required questions, they can submit the form to move it to the next step, if applicable.
